A seemingly routine childhood fall can sometimes conceal life-threatening internal injuries. Recently, the Department of General Surgery at Hai Phong Children’s Hospital performed an emergency laparoscopic operation, successfully treating a 7-year-old boy who sustained a rare duodenal rupture caused by a bicycle mishap.
Progressive Abdominal Pain After a Minor Bicycle Accident
According to hospital reports, the young boy experienced abdominal discomfort shortly after falling off his bicycle. Over the following hours, the pain grew progressively worse. Although his family sought medical attention at multiple local clinics, the root cause of his worsening distress remained elusive during initial checks.
Upon arriving at Hai Phong Children’s Hospital, medical specialists acted swiftly, conducting comprehensive clinical examinations and specialized diagnostic imaging. Scans quickly revealed severe internal trauma: a ruptured duodenal bulb accompanied by secondary peritonitis—a dangerous inflammation of the abdominal lining caused by leaked digestive enzymes.

Pediatric surgical specialists highlighted that duodenal perforation resulting from blunt abdominal trauma is exceptionally uncommon in children. Because the duodenum is positioned deep within the retroperitoneal cavity and shielded by surrounding organs, trauma indicators may remain masked immediately after an impact. However, once ruptured, the leakage of bile and digestive fluids rapidly triggers severe peritonitis, escalating the risk of systemic infection and septic shock if left untreated.
Minimally Invasive Emergency Surgery Preserves Digestive Function
Faced with ongoing intra-abdominal contamination and acute peritonitis, the surgical team immediately initiated an emergency laparoscopic intervention to prevent further deterioration.

During the delicate procedure, surgeons repaired the perforation in the duodenal wall, thoroughly cleansed the peritoneal cavity to eliminate infectious fluids, and placed drainage tubes. This minimally invasive technique effectively brought the infection under control while safeguarding the child’s long-term gastrointestinal functionality.
The patient achieved an encouraging recovery post-surgery. His vital signs stabilized rapidly, and he was able to resume oral feeding by the fourth postoperative day. With his condition fully normalized, the boy has been scheduled for hospital discharge.
Recognizing Hidden Internal Trauma in Children
This case serves as a crucial reminder for parents and caregivers regarding the hazards of blunt abdominal trauma. Following incidents such as road collisions, bicycle falls, drops from heights, or heavy impacts to the torso, vital internal organs can suffer severe damage despite the absence of external wounds or obvious bruising.

Red Flag Symptoms Requiring Immediate Medical Attention:
- Escalating or persistent abdominal pain that worsens hours after an impact.
- Unexplained nausea or persistent vomiting following a fall.
- Abdominal swelling, rigidity, or severe tenderness upon light touch.
- Fever, profound lethargy, pale skin, or altered digestive habits.
Parents should never ignore lingering complaints after a physical accident. Prompt evaluation at a specialized pediatric medical center is essential to preventing irreversible complications.
