Folk medicine often promises vitality and natural detoxification, but some deep-seated beliefs can pave the way to catastrophic medical emergencies. A stark reminder of this reality occurred recently when a 49-year-old man was rushed to the Military Central Hospital 108 in Hanoi, suffering from acute, multi-organ damage after consuming a raw grass carp gallbladder mixed with alcohol.
The patient, who had a history of chronic alcohol consumption, swallowed the gallbladder during a social gathering. He acted on the widespread folklore that carp gallbladder serves as a powerful tonic to ‘cool’ the liver and enhance physical stamina. Instead of vitality, his body experienced a violent systemic shock. Within twelve hours, he developed relentless vomiting, severe diarrhea, a mild fever, and extreme exhaustion. Clinical examinations at the hospital revealed highly elevated liver enzymes and dangerously spiked triglyceride levels, signaling severe acute liver injury.
According to Dr. Hoang Nghia Quoc from the Department of Hepatobiliary and Pancreatic Treatment, the culprit behind this rapid deterioration is a chemical compound called 5α-cyprinol sulfate, found abundantly in the bile of the Cyprinidae (carp) family. This compound acts as a direct cellular poison targeting both the liver cells and the renal tubules in the kidneys.
Crucially, this toxin is highly thermostable. Traditional preparation methods—whether boiling the gallbladder, drying it, or soaking it in strong liquor—completely fail to degrade or neutralize the poison.
Medical experts outline the typical timeline of fish gallbladder poisoning in three dangerous phases:
– Early Phase (First few hours): Marked by severe gastrointestinal distress, including nausea, watery diarrhea, and abdominal cramping.
– Organ Injury Phase: Characterized by jaundice, soaring liver enzymes, blood clotting issues, and potentially a drop in blood pressure or altered consciousness.
– Delayed Renal Phase: Often occurring after the stomach symptoms seem to improve, this phase leads to acute kidney injury, decreased urine output, and fluid retention. In many instances, patients require immediate hemodialysis to survive.
Because there is no specific antidote for 5α-cyprinol sulfate, treatment is strictly supportive, focusing on intensive hydration, managing organ failure, and filtering toxins through dialysis if necessary. Healthcare professionals strongly urge the public to abandon the hazardous practice of ingesting animal gallbladders in any form, warning that this traditional ‘cure’ is nothing short of a chemical hazard.
